Faith-based health system implements integrated healthcare for optimal patient outcomes
The Challenge
A regional, faith-based healthcare system aspired to shift towards an Integrated Care model. This integrated approach required new relationships between behavioral health, addiction, and primary care professionals. These professionals had no existing system of collaboration and limited experience interfacing with hospital administration.
The Solution
The client invited Third River Partners to design a custom experience to form a Steering Team of integrated care professionals. This team developed shared goals and key business strategies to achieve them. Once shared goals were realized, Third River facilitated a number of sessions to aid the team in designing a model for delivering patient-centered care. Third River connected the client with experts in the field for benchmarking calls and site visits to support the new model’s implementation.
Third River worked with client to define the population the new model would serve. Together, Third River and the client identified key stakeholders and opportunities for funding and partnership in the community.
The Outcome
The highly engaged Steering Team, alongside Third River, created a business plan to fulfill shared goals of the new integrated care model. This core document serves as the basis for the next steps of increasing organizational engagement around integrated health, securing funding, and selecting community partners for collaboration.
